How to get the Delivery Instructions Slip (DIS) booklet for NSDL demat account?

You can obtain your DIS booklet by submitting a request form either online or offline. You must also send the addition of beneficiary details form (PDF) to Zerodha along with your DIS request.

Online requests

You can submit the DIS request form (PDF) online if you are an individual account holder and your Aadhaar is linked to your mobile number.

To submit your request online:

  1. Download and print the DIS request form (PDF).
  2. Fill out the details and sign the form.
  3. Scan the form and eSign it.
  4. Create a ticket and attach your e-signed DIS request form, ensuring it is vertically aligned.

You must submit your e-signed forms within 30 days from the date of eSign.

Offline requests

You must follow the offline method if your Aadhaar is not linked with your mobile number or if you are a non-individual account holder.

To submit your request offline:

  1. Download the DIS request form (PDF).
  2. If you are requesting the DIS booklet for the first time, enter your NSDL DP ID on the form. You can find your NSDL DP ID under the secondary demat section in Console. If your DP ID is not visible, create a ticket to request your NSDL CMR copy.
  3. Fill out all required details and sign the form.
  4. Courier the completed form to:

How to fill out the DIS request form

Your NSDL demat account will have one of these DP IDs: IN304287/ IN300095 / IN302871 / IN303446

Collection instructions:

  • Collected by hand: Select this option if you plan to visit the Zerodha head office to collect your DIS booklet. You must submit a self-attested copy of your ID proof along with the DIS request form.
  • Additional booklet: Select this option if you have lost Annexure B from your existing DIS booklet and need an additional booklet. If you still have Annexure B, you can obtain an additional booklet by sending Annexure B to the head office without selecting this option.
  • Not required to cancel unused slips: Select this option if you do not want your existing DIS slips cancelled. Create a ticket to confirm this preference.
  • Required to cancel unused slips: Select this option if you want your existing DIS slips cancelled. You need to mention the slip number and select one option: Lost, Misplaced, or Others (mention the reason if you select Others).
  • To cancel DIS slips/book: Select this option to cancel your DIS slips without requiring additional booklets.
  • By authorised representative: Select this option if your representative will visit the head office instead of you to collect the DIS booklet. Your authorised representative must submit the DIS request form and ID proof, both signed by all account holders. This requirement applies to non-individual account holders as well. Ensure your account is not dormant when your representative visits Zerodha. If it is dormant, we will conduct a call verification or an In-Person Verification (IPV) with you to authenticate your representative.
  • Correspondence address: Select this option if you are applying for a new DIS booklet and want Zerodha to deliver it to your address. We will send your DIS booklet only to your communication address registered with Zerodha.

Additional requirements:

  • Include names and signatures of all account holders.
  • Non-individual account holders must provide seal and signature on the DIS request form.

Costs and limits

  • Your first DIS booklet with 10 slips is free.
  • Additional booklets cost ₹100 + 18% GST per booklet, plus ₹100 + 18% GST courier charges.
  • You can transfer a maximum of 5 securities (ISINs) per DIS. Use the DIS Annexure form (PDF) to transfer more than 5 securities.
